How to Register
New Registration (Joining the Practice)
If you wish to register at this practice you will need to produce the following current documents:
- Your passport or UK photographic Drivers Licence
and
- Two recent utility bills (which may include the current years Council Tax, Electricity, Water or Gas, or a letter from your landlord or housing department to confirm your residency). Documents must be dated within the last 3 months.
These documents will be copied and retained in your medical file.
Failure to provide documentation may result in the refusal of your
registration.
